Invisalign treatment prescription forms started after 12am July 1st, 2018 (Pacific Standard Time) will be on new Invisalign treatment options and
will follow the Invisalign Pricing,Terms and Conditions, Invisalign treatments with prescription
forms submitted prior to July 1st, 2018 will not be affected. Invisalign treatments with prescription forms started
prior to July 1st 2018 but unsubmitted before August 30st 2018 will be cancelled.
